Hi Bob and Nori Nakamura: I am attaching a copy of the J-1 request
form. You can fill out the information ahead of time and bring it to
the meeting if you like, or send it to me by email. I will need a
letter of offer from the appropriate faculty member (Macosko? Francis?
someone else?) and a non-sponsored budget number from that individual.
I will need dates that the individual plans to arrive at the U and
depart the University. I need a copy of the individual's passport ID
page (with the photo on it). Email copies work fine; fax does not work
well. If the individual is bringing dependents I need their passport ID
page as well, and all the information requested on the page regarding
dependents accompanying the primary J person. I have to request IDs for
the primary traveler and for the dependents so I need precise names,
dates of birth, city of birth, country of birth, citizenship of each
individual, and permanent residence address. I will also need on
official letterhead from the appropriate office in your company a letter
that indicates how much support (in US dollars) will be provided to the
visitors who are coming to the US for the length of their stay. A few
lines about the research they intend to work on while they are here
could be either in the letter of invitation from our faculty member or a
separate letter from the visitor or a company official. That is all I
can think of at the moment. If I've left anything out I'll make sure to
add it later, but I think this is quite thorough. Please feel free to
